The new IGRA, QuantiFERON-TB Plus (QFT-Plus), is based on the enzyme-linked immunosorbent assay detection of IFN-γ after stimulation with Mycobacterium tuberculosis TB1 and TB2 antigens. TB1 elicits a cellular-mediated immune (CMI) response by CD4 T cells, and TB2 contains peptides recognized by both CD4 and CD8 T cells. QFT-Plus employs two antigen tubes (TB1 and TB2) for diagnosis of M. tuberculosis infection. The TB1 minus Nil value indicates the interferon-gamma response of CD4+ T lymphocytes, specifically stimulated by the TB1 antigens. The TB2 minus Nil value indicates interferon-gamma response of both CD4+ and CD8+ T lymphocytes, stimulated by the TB2 antigens.

Dec 26, 2019 for each antigen tube of the QFT-PLUS showed a good correlation with the IFN -γ release of the QFT-GIT (TB1 = 0.912, p < 0.001; TB2 = 0.918 Antigens specific for M. TB 1.

The way we interpret it is, there should be a difference of more than O.35 IU/ml, between TB antigen ( 0.479 in your case ) and nil antigen (0.040 ). However in absence of any symptoms, it seems to be due to latent TB or can be a false positive report. Confirm volume in all 4 tubes (Grey top NIL tube, Green top TB1 ANTIGEN tube, Yellow top TB2 ANTIGEN tube and Purple top MITOGEN tube) is at or near the black fill mark. The black mark on the side of the tubes indicates the 1.0 ml fill volume. The blood collection tubes have been validated for volumes ranging from 0.8 to 1.2 mls.
